  • When your physician’s office schedules your procedure, you must complete the appropriate assessment form.  This may be completed online, or by using an HPSC mail-in assessment form from your physician’s office or the front desk of HPSC.  Upon completion of your assessment information, one of our nurses will contact you, review the information you have provided and give you additional information on what to expect the day of your procedure.  If you have not received a call two business days prior to your procedure, please call the Surgery Center  at (336) 878-6197 and leave a message with the best phone number and time of day to receive a return call.
  • Please note the online assessment can be submitted immediately.  However, if there is not sufficient time to complete and mail or hand deliver the assessment form for it to arrive two business days prior to your surgery, fill out the form and keep it with you to review with our assessment nurse by phone. 
  • Information on the assessment provides our staff with important aspects of your history so we are able to provide you with a quality outpatient experience.
  • Contact your physician’s office on the workday (Monday-Friday) before your surgery to find out what time your surgery is scheduled.  If you are having a pain management procedure, contact the Surgery Center at (336) 878-6068 for your scheduled time.  Plan to arrive at the time given, but no earlier than 6:15 am.
  • Bathe or shower before surgery.  You may brush your teeth and rinse your mouth, but do not swallow any water.
  • Please wear casual, loose fitting clothing that is comfortable and easy to change.
  • Wear glasses instead of contact lenses.  Be sure to bring a case for your glasses.
  • You may wear light make-up unless you are having facial or eye surgery.
  • Jewelry should not be worn the day of surgery.  All body piercing(s) and jewelry must be removed.  The Surgery Center is not responsible for lost or damaged valuables.
  • Please bring your current insurance card and method of payment.
  • Unless instructed differently, you should not have anything to eat or drink including water, gum or mints after midnight before your procedure.  This restriction is for your safety, decreasing the risk of nausea and vomiting and other serious consequences.  It is also to your advantage to drink liquids during the evening before surgery, up to midnight only.
  • Notify your physician if you suspect you are pregnant or have experienced any health changes since your last office visit such as a cold, flu, high fever or other signs of infection.  This may require that your surgery be postponed.
  • Be sure to arrange for a responsible adult (age 18 or older) to drive you home following surgery and assist you with postoperative instructions and care.
  • One parent or guardian must plan to remain at the Surgery Center during a child’s entire stay.
